Medical programs of the Orbita wellness complex

Уважаемые гости!
При заселении по путевке с лечением необходимо иметь при себе санаторно-курортную карту.

Чтобы её получить, необходимо обратиться к терапевту (педиатру) в поликлинику по месту жительства.
Согласно Приказу Минздравсоцразвития РФ № 99/227 от 22.12.1999 г., заполняется бланк формы 072/у-04 для взрослых, а для детей - форма 076/у-04.
При отсутствии санаторно-курортной карты возможно оформление ее в медицинском подразделении АО «ОК «Орбита» за дополнительную плату (согласно прейскуранту).
Без санаторно–курортной карты лечебные мероприятия осуществляться не будут!
